I hereby give my permission for the child noted above to participate in the tryouts for the Central
Florida Suns. I understand that participation may result in injury and protective equipment does not prevent all injuries. I do hereby waive, release, absolve and agree to hold harmless the Central Florida Suns, coaching staff, volunteers and participants.
